PREPARING FOR YOUR FIRST VISIT

We look forward to meeting you. Completing the required steps before your appointment allows our clinical team to review your information and protects the time reserved for your care.
All intake steps—including medical history, consent forms, insurance details, financial information, and required credit-card information—must be completed by the deadline provided in your appointment communications.
Before your appointment
Complete every Phreesia intake section; upload requested identification and insurance information; bring or submit relevant records; prepare your medication list; and review appointment, cancellation, and financial policies.
Intake deadline
For appointments booked at least 72 hours in advance, required intake must be completed no later than 72 hours before the appointment. If booked within 72 hours, complete intake within 24 hours or by the deadline communicated by the office. Incomplete intake may result in cancellation so the time can be offered to another patient.
At your visit
Please arrive as directed in your confirmation. Tell the team if you need language assistance, mobility accommodation, or support understanding forms or instructions.
